A note for patients. Protected health information created or received in the course of your care is governed by our HIPAA Notice of Privacy Practices, which is provided at the practice and available on request. This policy covers information collected through this website.
Appointment booking takes place on a separate scheduling system operated by our practice management provider. Information you enter there is governed by that provider’s terms and by our HIPAA notice, not by this page.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
If you consent to receive text messages from us, message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for assistance. Opting out of text messages does not affect your care or your ability to book an appointment by phone. Full program terms are in our Terms of Service.

No method of transmission over the internet is completely secure, so we cannot guarantee absolute security. We keep information only as long as we need it for the purposes described here, or as required by law.
This website is not directed at children under 13, and we do not knowingly collect information from them through it. We are proud to care for children as patients, and information collected in that context is handled under our HIPAA notice with the involvement of a parent or guardian.
