• Information you give us. If you message us through the chat on this site, we collect your name, phone number or email address, and the content of your messages, so that we can reply.
• Information collected automatically. Standard web data such as IP address, browser and device type, pages visited and referring site. We use this to understand how the site is used and to keep it working.
• Cookies. This site uses cookies for basic functionality and analytics. Most browsers let you refuse cookies, and the site will still work.

Appointment booking takes place on a separate scheduling system operated by our practice management provider. Information you enter there is governed by that provider’s terms and by our HIPAA notice, not by this page.

How we use your information

• To respond to your questions and to contact you about your care or your appointment
• To send appointment reminders, recall notices and practice updates where you have asked to receive them
• To operate, maintain and improve this website
• To meet our legal and regulatory obligations

Text messaging and mobile information

If you consent to receive text messages from us, message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for assistance. Opting out of text messages does not affect your care or your ability to book an appointment by phone. Full program terms are in our Terms of Service.

How we share information

We do not sell your personal information. We do not buy or sell leads. We share information only with:

• Service providers who help us operate the practice and this website, such as our practice management, scheduling and communications platforms, under contracts that limit their use of the information
• Anyone you have asked us to share with, such as your insurer or another treating provider
• Authorities where we are required to by law

Security and retention

We use reasonable administrative, technical and physical safeguards to protect the information we hold. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security. We keep information only as long as we need it for the purposes described here, or as required by law.

Your choices

• Reply STOP to any text message to stop receiving them
• Use the unsubscribe link in any marketing email
• Contact us to ask what information we hold, or to correct or delete it, subject to the record-keeping rules that apply to healthcare providers

Children

This website is not directed at children under 13, and we do not knowingly collect information from them through it. We are proud to care for children as patients, and information collected in that context is handled under our HIPAA notice with the involvement of a parent or guardian.
